Rotary Bangalore ICICI Prudential Dialysis Centre:Providing succour to Diabetics 

 Rotary Bangalore ICICI Prudential Dialysis Centre initiative was set up on 27 June 2011 with the inauguration of five dialysis machines initially at RRMCAH. The centre  was inaugurated by PRID Pandurang Setty and PDG M.N Suresh along with a host of dignitaries from RRMCAH and RCB.

The Diabetic Care Centre was inaugurated on 20^th October 2010 at the Rotary Bangalore Muthapppa Attavar Centre, creating awareness on Diabetes with far reaching complications and one of them being end stage renal failure. It was this truth which paved the way of setting up a Dialysis Centre at RRMCAH.

  The Dialysis Centre provides subsidized dialysis to low-income patients priced at cost of operations,with an additional average subsidy of 25% .Diabetes is fast gaining the status of a potential epidemic in India with more than 62 million diabetic individuals currently diagnosed with the disease. It is predicted that by 2030 diabetes mellitus may afflict up to 79.4 million individuals in India. Further, the disease is prevalent in all sections of society. The problems are worsened at the lower socio-economic level, due to lack of awareness, access to health care and late diagnosis leading to vulnerable effects of the condition. Diabetes has been found to be the primary cause of kidney failure in nearly 45% of patients undergoing dialysis. It is necessary to spread awareness of, diagnose and provide ongoing medical support to patients suffering from kidney failure (also on account of being diabetic) belonging to the lower socio-economic strata. The area of Kumbalgodu around Raja Rajeshwari Medical College  and Hospital (RRMCH) on the south-west outskirts of Bangalore , with a population of around 200,000 is estimated to have around 25,000 diabetic patients who need such treatment.

In 2011, The Rotary Club of Bangalore  alongwith ICICI Prudential Life Insurance Company Limited engaged in a program along with RRMCH  in setting up “Rotary ICICI Prudential Dialysis Centre” at the RRMCH campus in Kambipura, near Mysore Road to cater to the  dialysis needy patients in the Kumbalgodu and adjoining areas. The program was undertaken to achieve the following objectives:To spread awareness of Diabetes;enable diagnosis and consequent treatment through an outreach program using a mobile van-cum-ambulance; and to provide dialysis to needy patients at negligible cost

 Raja Rajeshwari Medical College Hospital (RRMCH) has been providing healthcare and diagnostic services to people in the area of Kumbalgodu at subsidized rates patients for specialist treatment.

The key stake holders being ICICI Prudential Life InsuranceCompany Limited, RRMCAH and RCB.

A  Dialysis Program committee comprising members from all three key stakeholders has been constituted under the aegis of the Rotary Club of Bangalore Charitable Trust (RCBCT). The committee meets frequently and discusses the performance of the program whilst exploring options to expand the reach of it. In specific, the Committee is also responsible for funds mobilisation, project monitoring and audits and subsidy allocation 

The Rotary Club of Bangalore was also successful in raising close to INR 29 lakhs  by way of a ” Dinner for Charity ” program .These funds have been used to provide an additional five Dialysis machines , in addition one more being through the benevolence of one of our RCB members, taking the number to 21. In addition a corpus of INR 5 laķhs has been donated and the interest accrued provides further subsidized dialysis for the very  poor.

• We began our project by charging INR 600 per dialysis which was at the operational cost of the hospital. 
• The fees charged is less than half the rate a regular hospital may charge for their dialysis. 
• Due to higher operational cost and inflation we are currently charging INR 750 per dialysis.
• We incur an under-recovery of 25% of the dialysis fees charged on a per dialysis basis.
• Our maintenance cost is at INR 20,125/Machine/annum inclusive of service tax.
• Beyond these costs, we provide  tests and scan for patients regularly. The initial costs for such an investigation is INR 1500/patient. Regular investigation costs average  INR 400/month.

• Every patient needs two Dialysis per week, and every machine can serve seven patients per month.
•  The Project began in Jun 2011, and for the six months under operation in 2011, we provided 600 dialysis and served 13 Patients. 
• Cumulatively we have conducted over 54000 dialysis till date, and are currently serving 130 patients for dialysis.

• The cost of a dialysis machine is INR 6,25,000. When we began operations in 2011, the cost of a machine was INR 6,00,000 
• Further to the cost of the machine, we spend on additional infrastructure like the Reverse Osmosis Plant, Oxygen piping, Dialyser Reprocessing Machine or Polar Cots and Mattresses which on a per machine along with miscellaneous charges comes to INR 1,25,000 as of 2016.
• Ambulance and Outreach Van was purchased to help spread awareness to help patients requiring diabetes care and transporting them to and from the hospital during care.
• The cost of the Ambulance and Outreach Van has totalled INR 24,00,000. 
• The total initial investment in 2011 was INR 59,00,000 including the cost of five machines purchased initially.
• Since beginning operations we have purchased additional five machines each in 2013 and 2016, totalling 15 machines, out of which one is used in ICU and one is for Hepatitis C patients, and 13 machines are used for dialysis of regular patients. Now six more machines have been added taking the total to 21 machines.

 We generate at least one additional employment opportunity when we buy a new machine. We employee Nurses, Technician, Attendees, Assistants, and till date we have 20 employees excluding the Nephrologists. The costs of running the centre costs around INR 36,00,000 per annum.

• Further, we save  numerous patients’ monthly income which they would have had to forgo if the dialysis wasn’t performed. We have assumed that in 2011 the patients we served earned an average income of INR 25,000 per month
• Through our project we generate employment, provide a cheaper treatment and further we save an individual’s income, which he would have to forgo in case he/she did not undergo treatment. Through the Cost/Benefit Analysis, we find that our project till date has created a Social IRR of 95% on financial investment.
• Our Per Machine IRR is at 144% for the present capacity assuming average life over the long term (8 years).
